Program Personnel Changes

For a complete description of key program personnel policies, please refer to Section 6.0 of CoARC's Accreditation Policies and Procedures Manual.

 

 Change in President/CEO

 

Complete and submit the Change in President/CEO form

 

 Change in Dean

 

Complete and submit the Change in Dean (or comparable administrator) form

 

 Change in Program Director (Refer to Standards 2.03, 2.05-2.07)

 

Complete the following seven (7) steps:

 

1.  Complete this "Change in Program Personnel" Template.  

2.  Provide a copy of the institution's Letter of Appointment and Acceptance. 

    The letter(s) must contain at least the following (4) items:                                 

            1. Signature of the President/CEO or Dean/Administrator                      
            2. Position Title
            3. The effective (start) date
            4. A guarantee of full-time faculty appointment


NOTE: If you do not have a "Letter of Appointment and Acceptance"  and need to create one, you may use this template.
 
3.    Provide a copy of the PD's curriculum vitae (CV)

4.    Provide a copy of the PD's valid state license;

5.    Provide a copy of the PD's valid NBRC RRT certificate.

6.    Provide a copy of the PD's academic transcript (baccalaureate or higher)

7.    Return all six (6) documents to:

 Change in Director of Clinical Education (Refer to Standards 2.03, 2.10-2.12)

 

Complete the following seven (7) steps:

 

1.  Complete this "Change in Program Personnel" Template.  

2.  Provide a copy of the institution's Letter of Appointment and Acceptance. 

    The letter(s) must contain at least the following (4) items:                                 

            1. Signature of the President/CEO or Dean/Administrator                      
            2. Position Title
            3. The effective (start) date
            4. A guarantee of full-time faculty appointment


NOTE: If you do not have a "Letter of Appointment and Acceptance"  and need to create one, you may use this template.
 
3.    Provide a copy of the DCE's curriculum vitae (CV)

4.    Provide a copy of the DCE's valid state license;

5.    Provide a copy of the DCE's valid NBRC RRT certificate.

6.    Provide a copy of the DCE's academic transcript (baccalaureate or higher)

7.    Return all six (6) documents to:

 Change in Medical Director/Co-Medical Director (Refer to Standard 2.14)

 

 

Complete the following six (6) steps:

 

1.  Complete this "Change in Program Personnel" Template.  

2.  Provide a copy of the institution's Letter of Appointment and Acceptance. 

    The letter(s) must contain at least the following (3) items:                                 

            1. Signature of the President/CEO or Dean/Administrator                      
            2. Position Title
            3. The effective (start) date 


NOTE: If you do not have a "Letter of Appointment and Acceptance"  and need to create one, you may use this template.
 
3.    Provide a copy of the MD's/Co-MD's curriculum vitae (CV)

4.    Provide a copy of the MD's/Co-MD's valid state license;

5.    Provide a copy of the MD's/Co-MD's valid Board certificate(s).

6.    Return all five (5) documents to:

 Change in Primary PSG Instructor (Refer to Standard 2.16; Policy 6.011E)

 

Complete the following seven (7) steps:

 

1.  Complete this "Change in Program Personnel" Template.  

2.  Provide a copy of the institution's Letter of Appointment and Acceptance. 

    The letter(s) must contain at least the following (4) items:                                 

            1. Signature of the President/CEO or Dean/Administrator                      
            2. Position Title
            3. The effective (start) date
            4. A guarantee of a faculty appointment


NOTE: If you do not have a "Letter of Appointment and Acceptance"  and need to create one, you may use this template.
 
3.    Provide a copy of the PPI's curriculum vitae (CV)

4.    Provide a copy of the PPI's valid state license (if applicable);

5.    Provide a copy of the PPI's valid NBRC SDS certificate and/or valid BRPT RPSGT certificate.

6.    Provide a copy of the PPI's academic transcript (associate or higher)

7.    Return all six (6) documents to:

 Change in Billing Contact

 

Complete and submit the Change in Billing Contact form

 

 Change in Program Name

 

Complete and submit the Change in Program Name form


Note:  If this name change is associated with a change in sponsorship/ownership/legal status, an Application for Substantive Change must be completed.  Refer to CoARC Accreditation Policies 1.08, 9.01 and CoARC Standard 1.08 for more details.

 

 Change in Program Address

 

Complete and submit the Change in Program Address form

 

Note:  When the program changes its campus location and/or teaching facilities (i.e. lab, classrooms, etc.), an Application for Substantive Change must be completed.  Refer to CoARC Accreditation Policy 9.11 and CoARC Standard 1.08 for more details.
