Director of Clinical Education

 

 

 

 

Instructions for Change in Director of Clinical Education:

 

Please complete the following seven (7) steps.

 

1.    Complete this "DCE Change in Program Personnel" Template and put on your Program Letterhead. 

 

2.    Provide a copy of the institution's Letter of Appointment and Acceptance. 

 

       This letter must contain the following (4) items:    

                                  

bulletSigned by the President/CEO -or- Dean/Administrator 
bulletPosition Title
bulletThe effective (start) date
bulletA guarantee of full-time faculty appointment

 

         NOTE: If you do not have a  "Letter of Appointment and Acceptance"  and need to create one, you may use this template.

 

3.    Provide a copy of the Director of Clinical Education's Resume.

 

4.    Provide a copy of the Director of Clinical Education's State License.

 

5.    Provide a copy of the Director of Clinical Education's RRT Certificate.

 

6.    Provide a copy of the Director of Clinical Education's college transcript (baccalaureate or higher)

 

7.    Please mail all six (6) completed  documents to:
