Instructions for Change in Polysomnography Primary Instructor:

 

Please complete the following six (6) Steps.

 

1.    Complete this "Change in Program Personnel" Template and put on your Program Letterhead.  

 

2.    Provide a copy of the institution's Letter of Appointment and Acceptance. 

 

       This letter must contain the following (4) items:        

                             

bulletSigned by the President/CEO  -or-  Dean/Administrator                      
bulletPosition Title
bulletThe effective (start) date
bulletA guarantee of faculty appointment

 

        NOTE: If you do not have a "Letter of Appointment and Acceptance"  and need to create one,

        you may use this template.

 

3.    Provide a copy of the Primary Instructor's Resume (showing required clinical experience).

 

4.    Provide a copy of the Primary Instructor's RPSGT Certificate.

 

5.    Provide a copy of the Primary Instructor's college transcript (associate degree or higher)

 

6.    Please mail all five (5)  documents to:
