Instructions for Change in Program Director:
Please complete the following seven (7) Steps.
1. Complete this "Change in Program Personnel" Template and put on your Program Letterhead.
2. Provide a copy of the institution's Letter of Appointment an Acceptance.
This letter must contain the following (4) items:
NOTE: If you do not have a "Letter of Appointment and Acceptance" and need to create one, you may use this template.
3. Provide a copy of the Program Director's Resume.
4. Provide a copy of the Program Director's State License
5. Provide a copy of the Program Director's RRT Certificate.
6. Provide a copy of the Program Director's college transcript (baccalaureate or higher)
7. Please mail all six (6) documents to:
